Writing the survey takes five minutes. Getting forty of the right people to answer it, and knowing what the number means once they have, is the part that kills you. That is the part we do.

The tool runs the survey. The diagnosis gets you the answer. Writing the survey is the five-minute part. Working out who counts as engaged, getting forty of them to reply, and knowing what the number means once they have is where it usually stops. If you want to run it yourself, buy the tool, it is $99. This is for when you would rather it were done.
Then a survey cannot tell you anything honest, and we run the interview-based version instead: 8 to 12 customer interviews, synthesised into an ICP definition, a build-or-pivot verdict and the same prioritised roadmap. You get no score, on purpose. A percentage from 12 responses is a guess wearing a lab coat.
We will not hand you a score we do not believe. Below roughly 40 valid responses, a Sean Ellis percentage is not a measurement, it is noise, and giving you one anyway would be the most expensive thing we could do to you. So you get what the sample genuinely supports: the qualitative read, the ICP signal, the barrier themes from the answers we did collect, and a written note on exactly what it would take to reach a reliable number. Your target sample is agreed at intake against your real user base, which is why this is rare rather than a surprise.

Yes. Every engagement includes the PMF Startup Kit licence with lifetime access, and you keep it when the engagement ends. Re-run the survey as often as you like afterwards.
